1. How can I obtain services?
a) Have a documented or established disability. If you do not have a copy of your documentation, you can request a letter from your doctor or have your doctor complete a Client Information Form.
b) Complete the registration process in Disability Services (DS). The registration form can be downloaded online.
c) Submit the registration form and documentation of your disability to Disability Services (DS) and meet with a DS Counselor.
d) Obtain an Accommodation Card from Disability Services.
Contact Disability Services for a consultation and counseling.
DS does not have a tutoring program; however, DS has a list of organizations and labs that offer tutoring.
a) Complete the Transportation Golf Cart Program application in Disability Services and meet with the Transportation Golf Cart Program Coordinator.
b) Eligibility is determined by the severity and duration of the student's disability
c) Due to limited space on the carts and a lack of staffing, a limited amount of persons will be allowed into the program. The Transportation Golf Cart Program is not available in the summer.
Contact Disability Services and speak with the Aretha Fulcher who is the Deaf and Hard of Hearing Coordinator.
Yes, contact Disability Services at the end of January each year. The contact person is Ms. Sondra Sims in Disability Services.
Student volunteers share their lecture notes with students who are Deaf and Hard of Hearing, visually impaired, or have learning disabilities, ADD, ADHD, and physical disabilities.
All disability parking spaces in the garage are free of charge for people with state disability parking and who have registered with Disability Services. Disability parking is also available in any disability space on campus except for contract lots. The disability spaces in the contract lots have to be purchased. Spaces designated as faculty/staff and/or student spaces are also available to students with state disability parking. If you have any questions or need clarification, the contact person is Vaneta Harvey in Disability Services.
Students may apply for temporary state disability parking. To apply for a temporary state disability parking hangtag a form which is available on the DS website must be completed by a doctor. The completed form must be taken to the Revenue Department. The form can be found on the DS website under "DS A-Z", "Parking" and click on certification form which is in red print. If students do not want to apply for a temporary state disability parking hangtag, they may contact the ASU Student Health Center for a placard which allows the student to park in faculty/staff and student parking spaces only.
Visitors with state disability parking can park in the spaces marked disability spaces on any level of the parking garage free of charge.
11. What is the Technology Fair?
Disability Services holds a Technology Fair every other year to showcase technology used by students and technology useful for persons with disabilities. For more information about the Technology Fair go to the DS home page and click on "Events" then scroll to the bottom of the screen and click on "Technology Fair".
They are recommendations to web authors on how to create accessible web content.
13. Do I need to make my website accessible?
Yes. All ASU websites including faculty members websites for their classes need to be accessible. For help in making websites more accessible, contact Christina Laurentia in Disability Services. Read about web accessibility guidelines.
Delta Sigma Omicron (DSO) is a student organization sponsored by Disability Services. To view more information about DSO, please visit the DSO website. Other volunteer organizations include: Volunteers in Action, Notetaking, Campus Guides, and Study Services.
15. I would like to use alternative format for my books. How do I request the service?
a) Notify the Reader Services Coordinator, Christina Laurentia, to request books.
b) Submit your class schedule and the title, author, edition and ISBN of the books as soon as possible.
c) Always remember to pick up the alternative books and the CD player (if applicable).
d) Return the books and the CD player to Disability Services at the end of the semester.
Assistive Technology is any item, piece of equipment or product system, whether acquired commercially off the shelf, modified, or customized, that is used to increase, maintain, or improve functional capabilities of individuals with disabilities. Disability Services has software and devices available for students to use. The software is available in several computer labs across campus. Some of the devices are available to check out to qualified students. Check with Disability Services regarding the availability of specific Assistive Technology. Visit the assistive technology page.
17. I am registered with Disability Services. How do I receive services for this semester?
Students are required to contact Disability Services at the beginning of each semester. Students are responsible for picking up their accommodation card and discussing their accommodations with their instructors. If students experience academic problems, they should contact DS immediately to resolve the problem.
