How to Request Alternate-Format Textbooks

1. EmailFill out the AssistiveAlternate TechnologyFormat &Textbook AccessRequest Coordinatorform. with the following:
   1. Proof of purchase/rental (receipt, Amazon invoice, etc.)
   2. The name of the book(s) you are requesting
   3. The ISBN-13 for EACH book
   4. Which class each1 book is= for
   1
   5. Student ID number
   form.
2. Sign and return the Copyright Acknowledgment form to the Assistive Technology and Access CoordinatorCoordinator. (thisThis will be emailed to you oncefor allyou to sign and return, or you can sign a copy in the information above has been provided).office.

Once we have the textbook file(s), we will share them with you via OneDrive. Make sure you download your files, as they will only be on OneDrive for a few days.
