How to Request Tax Form

Students

1. Go to my.AState.edu.

2. Enter A-State email address and password and click Login.

3. Authenticate using Duo Mobile.

4. Click Banner9 Self Service Student.

5. Click TaxView Notificationand Pay Bill.

6. Select Click TaxMy YearAccount fromin the drop-listtop list.left of the screen.

7. Click Statements.

8. Click the 1098-T Tax Statements tab.

9. Click the View button to the right of the tax year to download a PDF copy of the tax statement to your device.

Employees

1. Go to my.AState.edu.

2. Enter A-State email address and password and click Login.

For more information, Students will need to contact the Treasurer's Office and Employees will need to contact Human Resources.
