Setup Fee:  $40.00 Required

Late Fee:  $30.00 (assess 5 days after each missed payment)

Enrollment Period:  01/05/2026 - 03/16/2026

Payment Dates:  01/15  01/31  02/15  02/28  03/15  03/31  04/15  04/30

Setup Fee:  $40.00 Required

Late Fee:  $15.00 (assess 5 days after each missed payment)
