Payment Dates:  8/01/31  9/30 02/28  10/03/31  11/04/30

Setup Fee:  $40.00 Required

Late Fee:  $30.00 (assess 5 days after each missed payment)

Semi-Monthly

Enrollment Period:  08/04/202501/05/2026 - 10/15/202503/16/2026

Payment Dates:  8/01/15  8/01/31  9/02/15  9/30 02/28  10/03/15  10/03/31  11/04/15  11/04/30
