Late Fee:  $30.00 (assess 5 days after each missed payment)

Enrollment Period:  08/04/2025 - 10/15/2025

Payment Dates:  8/15  8/31  9/15  9/30  10/15  10/31  11/15  11/30

Setup Fee:  $40.00 Required

Late Fee:  $15.00 (assess 5 days after each missed payment)
