Arkansas State University offers employees the opportunity to purchase cancer insurance through USAble Life. Employees may purchase individual plans as well as family plans. Evidence of insurability is required.
Available Plans
USAble Life offers three levels of cancer plans that pay cash to employees diagnosed with cancer. The plans cover hospital confinements, radiation, chemotherapy, and surgery. Please review the USAble brochure and summary plan description for more information.
How can I enroll in Cancer Insurance?
Employees may enroll within 31 days of hire or during open enrollment.
How do I receive my wellness benefit through USAble?
- Complete the Wellness Benefit Request Form
- Return the Request Form to the Claims Department address listed in the top right corner of the form
